Thinking About End Of Life, advance care plan workshop

Having expereienced the sudden on-set of illness & subsequent death of a loved one who had not communicated any end-of-life wishes, I was motivated to create my own 'advance care plan.' I am here to encourage and support others to do the same. 

An advance care plan is a written record that includes your wishes, preferences, values and goals relevant to all your current and future medical care needs, including when we are close to death and after we have died. 

Hopefully our own death is a long way off, and still, it is wonderful to reflect on what makes our days meaningful, what brings us joy, and what our values and beliefs are. 

Your plan will be especially helpful if something unexpected suddenly happens or when you become very unwell, as you may not be able to express your wishes and thoughts at that time. Writing down our wishes…
